1. Intake Forms

Please complete the intake forms emailed to you before your appointment.

If you need to complete them on-site, kindly arrive 10–15 minutes early.

If your forms are not completed before your scheduled start time, the time spent completing them may be deducted from your treatment time.

Free parking is available in our parking lot. If all spaces are occupied, street parking is also available nearby.

We understand that plans can change. Please provide at least 24 hours' notice to cancel or reschedule.

$50 Fee

Applies to cancellations made with less than 24 hours' notice and missed appointments (no-shows).
